About this talk

Due to a variety of factors, preventable transfers from skilled nursing facilities (SNF’s) to the hospital emergency departments (ED) are recognized as creating unnecessary expenses and stress for residents and staff. This session will review a pilot program involving six SNFs in the Southern Tier of New York, where telehealth was used to reduce preventable ED transfers by nearly 30%.

Key Takeaways

  1. See how telehealth can be an effective means of mitigating workforce shortages to provide medical and behavioral health services in the SNF setting.
  2. Learn about a turnkey solution, tailored to meet particular SNF needs, that allows rapid deployment and ease of implementation of telehealth.
  3. See why effective training and support allows a telehealth solution to be enthusiastically accepted and embraced by SNF leadership, staff, and residents.
